Spotty hairloss can also be caused by vitamin/nutritional deficiencies, and it doesn't necessarily have to be related to his skin infection... did they do any blood work?

His diet doesn't sound horrible at all, actually... but there is definitely a LOT of different stuff in it.
Cut out any peanut butter treats... it's one of the most common allergy causes in dogs.
