does anyone have an owner surrender form that i can change and use??

I found one

I, ___________________, an adult, do hereby voluntarily and irrevocably give, donate, surrender and release to ________________, hereinafter referred to as _____________________, the following animal(s), hereinafter referred to collectively as "Animal(s)":

I willfully surrender all medical records and information pertaining to the Animal(s). CSAC has my permission to contact my veterinarian for any necessary information pertaining to my Animal(s), and I hereby consent to the release of any and all medical information by any medical provider.

Good thing to have. We found that out after getting shafted by owners more than once. :x

Yes, that's a useful form indeed!

Wouldn't it be a good idea to have this "Owner Surrender" form and an "Adoption Contract" form as a sticky at the top of the Rescue section?
